Venous Needle Dislodgement in Dialysis

The issue of Venous Needle Dislodgement (VND) has recently been on the radar of the dialysis community.  In a joint effort with Dialysis Education Services based in Bellflower, CA and Marea Enterprises Inc., Renal Business Today published an important piece on the topic of VND.  The paper titled, Venous Needle Dislodgement in Dialysis Clinic Settings discusses risk factors and best practices in an effort to mitigate this potentially serious issue.

These include: enhancing visualization of the venous needle, proper taping and fixation, employing asymmetric dialysis machine limits and effective staff:patient ratios, among other important steps.  A discussion on the importance of proper clinic protocols in addition to a heightened awareness of an open safety culture were also emphasized.

A study released 2011 titled Sharps Injuries among Employees of Acute Care Hospitals in Massachusetts, 2002–2007 , main premise was with respect to the decrease in sharps injuries by healthcare workers employing sharps with engineered sharps injury protections.    It is however notable that injuries rates of technicians and other healthcare workers accounted for 30% of over 16,000 needlestick injuries experienced at these hospital settings.

Marea Enterprises Releases Needle Safety Protocol titled:

A Recommended Universal Protocol for Preventing Needlesticks to Allied Healthcare Workers in Perioperative Settings Where Intraoperative Neurophysiological Monitoring with Subdermal Needles is Performed.

The protocol provides specific concrete recommendations on how to prevent needlesticks within intraoperative neurophysiological monitoring (IONM).  This protocol is the first of its kind within this healthcare sector where healthcare workers experience significant numbers of needlesticks.  As is the case within healthcare in general, IONM experiences high levels of underreporting with respect to needlesticks.

Needlestick injuries have decreased since the passage of the federal legislation in 2000, however there have been noted increases in unintended needlesticks in surgical settings.  See link.

This is the text of the National Needlestick Safety and Prevention Act enacted by Congress and signed into law on November 6, 2000 by then President Clinton.  The law subsequently amended the existing OSHA Bloodborne Pathogens Standard.
